The Complete Health Improvement Programme (CHIP) is a 10-week course delivered online by healthcare professionals with expertise in lifestyle medicine. It supports you to make changes to the way you eat, sleep, move and relax. These changes have been shown to improve your health and, in some cases, put long term conditions such as type 2 Diabetes into remission.

Who is eligible for this programme?
To be referred for this programme, you need to have a diagnosis of type 2 diabetes and be on any diabetes medication. You will also need to have access to IT equipment and be able to commit to the 10-week programme.

What happens on the programme?
The CHIP course starts with a blood pressure, weight check and blood test. This is used to check your blood sugars, blood fat levels and liver function. These tests will be done at your surgery as you would usually do for your annual health checks.
You will then be sent login details to begin the 10-week online course. The online hub includes videos, online activities, and knowledge quizzes.
You will also meet regularly with your coach to set goals and ask questions. Your coach will provide ongoing support throughout the course.
How can I access this programme?
There are limited places available to select GP practices within mid and south Essex. You will need to be referred by a healthcare professional at your local GP practice to register with the CHIP programme.
